About Your Visit
What should I bring to my appointment?
- Arrive a little early in case you need to update your personal information as you check-in.
- Bring your Photo ID, Insurance Card (if insured), and copayment (credit/debit card, check, or cash).
- Depending on the purpose of your appointment, you may need certain patients forms filled out.
- Your immunization record
- Your updated medication list
- Completed Electronic Demographic Form (for new patients)
Also bring any questions you may have for your provider. We look forward to seeing you!
Can I reschedule my appointment?
Yes, of course. If possible, please give us a 24-hour notice to reschedule an appointment.
Where’s the Medical Clinic entrance?
As you enter the Sheridan County Health Complex building, look for our receptionist to direct you.
