About Your Visit

What should I bring to my appointment?
  1. Arrive a little early in case you need to update your personal information as you check-in.
  2. Bring your Photo ID, Insurance Card (if insured), and copayment (credit/debit card, check, or cash).
  3. Depending on the purpose of your appointment, you may need certain patients forms filled out.

Also bring any questions you may have for your provider. We look forward to seeing you!

Can I reschedule my appointment?

Yes, of course. If possible, please give us a 24-hour notice to reschedule an appointment.

Where’s the Medical Clinic entrance?

As you enter the Sheridan County Health Complex building, look for our receptionist to direct you.
